    Take the Step - Agency Participation Form

    BARRIERS ADDRESSED BY YOUR ORGANIZATION DURING AND AFTER THE EVENT
    Which of these broadly defined barriers will you assess or address during and after the event?
    1. Personal and Financial - Barriers from a lack of money, housing, healthcare, childcare, transportation, or other essential needs.
    2. Emotional or Physical - Barriers from physical or emotional limits or problems.
    3. Career Decision-making and Planning - Barriers from poor career or life planning or misinformation.
    4. Job-seeking Knowledge - Barriers from a need for improved job search and interviewing skills.
    5. Training and Education - Barriers from a lack of training or education.

    SERVICES PROVIDED BY YOUR ORGANIZATION DURING THE EVENT
    Examples: Assessments of literacy, job-skills, career aptitude, health, etc., resume' preparation, mock interviews, assistance with forms/applications.
    This would expedite access to services on the day of the event.

    PREPARING FOR THE EVENT
    • Setup will take place on Tuesday, October 22, from 8:30 AM to 9:30 AM
    • Sign in and Orientation: All agency staff and volunteers will Sign In and participate in Orientation at 9:30 am.
    • Doors open: Doors will be opened to participants at 10 AM and will close at 2 PM. 
    • Breakdown will take place from 2:00 to 3:00 PM.  
    We will provide one (1) table. If you are conducting assessments,mock interviews, etc. you might need additional table(s).
    For your staff, volunteers, and for participants.
    We will provide t-shirts to each staff member and volunteer who actively participates at the event. We will contact you at a date closer to the event to get t-shirt size requirements.

    EVENT AGENCY DIRECTORY 
    An Agency Directory will be made available to event participants.
    ETHNN will provide marketing materials, website listing and links, tables, chairs, a name board, access to electricity and Wi-Fi, a hospitality room, and t-shirts for agency staff and volunteers.

    Your logo may be included in marketing materials, including ETHNN website, signage and Agency Directory.
